• If a student requires medication, they should do this at home whenever possible. However, in the event a student needs to take medication during the school day, the following guidelines must be adhered to in order to comply with the Wisconsin Statue on Administration of Drugs to Pupils and Emergency Care.

    1. If a student requires over-the-counter medication at school, parental consent is required. Over-the-counter medications must be sent to school in the original manufacturer's container. The package must list the ingredients and recommended therapeutic dose in a legible format. Please complete: Non-Prescription Authorization Form

    2. If a student requires prescription medications at school, both parental consent and physician authorization are required. Prescription medications must be supplied in the original pharmacy labeled package. The package must list the name of the student, the name of the prescriber, the name of the prescription medication, the prescribed dose, the effective date, and the administration directions in a legible format.     3. No medication will be administered by school personnel until the consent forms are completed and on file with the school. Medication Authorization forms will be kept and stored confidentially.

    4. Medications designated as emergency medications, may be carried/self-administered at school if the student's physician, parent, and school nurse are in agreement. These include medications for asthma and allergies/anaphylaxis. Students who carry medications that will be self-administered must have a medication authorization form on file at school.

    5. Parents are responsible for bringing medication to school and picking up unused medication within 10 days after the medication is discontinued and/or at the end of the school year. Any medication remaining at school at the end of the school year will be properly disposed of.

    6. A request to administer nonprescription medication in a dosage other than the recommended therapeutic dose must also be accompanied by the written approval of a medical provider.
